Relevance: The long-term consequences of COVID-19 for women’s reproductive health remain insufficiently understood, despite increasing evidence regarding the impact of SARS-CoV-2 on hormonal regulation, ovarian reserve, and structural changes in pelvic organs. The study aimed to summarize, systematize, and critically analyze current evidence on the long-term impact of COVID-19 on women’s reproductive function, including hormonal, structural, and functional alterations. Materials and Methods: A literature review was conducted in PubMed, Scopus, and Embase (2020–2025) that included peer-reviewed studies on menstrual function, ovarian reserve, endometrial changes, pregnancy outcomes, and potential mechanisms of reproductive impairment. Results: Post-COVID women demonstrate menstrual irregularities, decreased AMH levels, endocrine dysfunction, signs of oophoritis, and endometrial abnormalities; pregnant women show higher risks of complications and placental disorders. Conclusion: COVID-19 exerts a complex impact on female reproductive health, highlighting the need for clinical monitoring algorithms and further prospective research.
