Relevance: Preparing women of fertile age with antiphospholipid syndrome (APS) for pregnancy is an important component of a favorable pregnancy course and of achieving live birth as the main goal of antenatal care. The study aimed to investigate the influence of pregravid preparation on the dynamics of immunological parameters of women of fertile age with antiphospholipid syndrome. Materials and Methods: The study was conducted among women of fertile age with APS, a total of 95 women. The women were divided into 2 groups: the main group-49 women with pregravidal preparation by using SET, and the comparison group-traditional pregravidal preparation, 46 women. In the main group, pregravid preparation included an examination for APS systemic enzyme therapy with the consent of the women. In the comparison group, traditional preparation for pregnancy was carried out. The control group included 40 healthy women of fertile age. Results: After pregravid preparation, both groups reliably decreased the number of circulating immune complexes (CICs); the main group achieved the control level. In the main group, a significant decrease in tumor necrosis factor α and a decrease and leveling of differences with the control in IL-2 and IL-4 levels were revealed. Both groups achieved a decrease in the detection of lupus anticoagulant (LA) and anticardiolipin antibodies (aCL) as APS markers, with a significant decrease in the main group. Conclusion: This study of immunological parameters of women with APS indicates normalization in cases of preconception preparation with SET.
