Predictive model for cesarean section following labor induction at full-term pregnancy

Objective. To develop a predictive model for cesarean section following labor induction with low-dose oral misoprostol in women with unfavorable cervical condition at full-term gestation considering clinical and anamnestic factors. Patients and methods. A retrospective cohort study was conducted based on the analysis of labor induction histories between January 2021 and December 2022. A binomial logistic regression including important independent variables was used to develop a predictive model. Results. According to the results of multiple logistic regression, the risk of cesarean section after labor induction was significantly associated with the following variables: age (AOR: 0.91; [0.85–0.97], p = 0.004), gestational age (AOR: 0.78; [0.64–0.97], p = 0.022), Bishop score (AOR: 0.29; [0.20–0.44], p < 0.001), BMI (AOR: 1.15; [1.06–1.26], p < 0.001). Conclusion. The proposed model can be used as an objective tool to assess the risk of cesarean section in women with unfavorable cervical condition who underwent labor induction with low-dose oral misoprostol. This will allow to stratify the risks in this category of pregnant women and choose the optimal mode of delivery considering their individual characteristics. Further, it is planned to develop a convenient calculator for practical application.
