Morphofunctional immaturity of the fetus: multidisciplinary approaches to diagnosis and management of full-term and premature newborns (A literature review)

Relevance: The article examines current concepts of morphofunctional immaturity of the fetus, observed in both preterm and full-term newborns. A comparative analysis of clinical features was conducted, identifying the main etiological factors, pathogenetic mechanisms, and modern approaches to diagnosis and treatment. Special attention is given to multidisciplinary strategies for managing newborns with signs of immaturity. The necessity of early detection of pathological conditions and comprehensive prevention within the motherplacenta-fetus system is emphasized. The study aimed to identify the features of the formation and clinical course of fetal morphofunctional immaturity from the perspective of interaction within the mother–placenta–fetus system in order to substantiate interdisciplinary approaches to its diagnosis and management in term and preterm newborns. Materials and Methods: A literature review was conducted using the databases e-Library, Google Scholar, and PubMed with the keywords: newborn, full-term newborn, and functional immaturity. Results: Similar morphological changes (disembryogenetic structures, signs of fetoplacental insufficiency, vascular anomalies) were identified in the placentas of both preterm and full-term infants with signs of immaturity. Statistically significant differences were found in the levels of several trace elements. Based on this literature review, multidisciplinary approaches to managing newborns with gestational-age-related morphofunctional immaturity were summarized, including early neonatal diagnostics, collaboration among obstetricians-gynecologists, neonatologists, and pathologists, and personalized strategies for postnatal follow-up and correction of metabolic disorders. Conclusion: The findings confirm the need to broaden the concept of “morphofunctional immaturity of the fetus for gestational age,” taking into account factors beyond gestational age, and to implement an interdisciplinary approach (obstetrician-gynecologist, neonatologist, pathologist) to diagnosis and management. The placenta can serve as an informative marker of intrauterine distress.
