Preventive Measures of Primary Arterial Hypertension in Children and Adolescents

Purpose. To study risk factors for primary arterial hypertension (AH) in children and adolescents for timely implementation of preventive measures. Materials and methods. A retrospective analysis of case histories (25) and outpatient records (36) of children and adolescents aged 13 to 17 with high blood pressure episodes and arterial hypertension (AH) underwent examinations and treatment at JSC SCPPS in 2015-2021 carried out. Patients with symptomatic/secondary hypertension were excluded from the study. Depending on the level of blood pressure increasing, the patients were divided into several groups: Group I - with blood pressure increased in the range from 120/80 to 129/80 mm Hg - 24.6% (15), Group II - with 1st degree AH (from 130/80 to 139/89 mmHg) - 54.1% (33), Group III - with 2nd degree AH (from 140/90 mmHg and higher) - 21.3% (13). Results. The majority (96.7%) of children and adolescents with primary AH showed hereditary burden of cardiovascular and endocrine diseases (95.1%), as well as associated risk factors for primary AH. A clinical follow-up of children with primary arterial hypertension is given. Conclusions. In children and adolescents with primary AH, associated cardiovascular risk factors were observed, such as: sympathicotonic type of autonomic dysfunction (68.6%), overweight (47.5%), 1st degree obesity (32.8%), 2nd degree obesity (13.1%) and 3rd degree obesity (1.6%), physical inactivity (95.1%), pathological character and personality features (77.0%), increased dietary salt consumption (34.4%), and dyslipidemia (18.0%). Primary AH in children and adolescents was mainly characterized by increased systolic blood pressure and increased left ventricular myocardium mass in all patients with 2nd and 3rd degree obesity and in 1/3 of those with 1st degree obesity.
