Relevance: Heavy menstrual bleeding (HMB) is a common gynecological problem among women of reproductive age, significantly affecting their quality of life. The study aimed to determine the optimal conservative method of treating heavy menstrual bleeding in women of reproductive age based on an analysis of clinical efficacy, safety, and impact on patients’ quality of life in real clinical practice. Materials and Methods: The study included two stages: a survey of 180 women, 33.3% of whom were diagnosed with HMB, and an evaluation of treatment outcomes using three methods: a combined oral contraceptive (COC) containing dienogest and estradiol, a levonorgestrel-releasing intrauterine system, and tranexamic acid. Results: All treatments under study demonstrated a positive clinical effect, but the greatest clinical efficacy was demonstrated by the COC containing dienogest and estradiol, which provided a more pronounced reduction in blood loss, stabilization of hemoglobin levels, and a positive effect on subjective indicators of quality of life. Differences in patient tolerability and satisfaction were also found between treatment methods. Conclusion: The study highlights the feasibility of using COC containing dienogest and estradiol as a first-line treatment for HMB in women of reproductive age.
