The impact of labor dystocia at first stage on postpartum and perinatal outcomes: a retrospective cohort study

Dystocia, also known as obstructed labor, is a complex condition in obstetrics that can be caused by abnormalities of uterine contractions, maternal pelvis, fetal size or position. Objective. To analyze adverse outcomes during the postpartum and perinatal period in patients with poor progress in labor. Patients and methods. This retrospective study included 503 pregnant women aged 18–38 years who delivered between January 2012 and December 2016. Two patient groups were identified: a group with a prolonged latent phase of the first stage of labor (n = 328) and a group with a prolonged active phase (n = 175). Results. Admission of pregnant women with cervical dilation of 2 cm (80% vs. 29%, p < 0.05) resulted in an earlier diagnosis of prolonged latent phase (62% vs. 18%, p < 0.05). There was a correlation between the diagnosis of a prolonged latent phase (dilation up to 4 cm) and the rate of cesarean section (62% vs. 18%, p < 0.05); the earlier the diagnosis was made, the higher the cesarean section rate was (42% vs. 5.4%, p < 0.05). With the diagnosis of prolonged labor, the number of labor complications (70% vs. 10%, p < 0.01) and neonatal asphyxia (1.6% vs. 0%) increased in the group with a latent phase with dilation up to 4 cm. In the group with a prolonged active phase with dilation of 4 cm, labor complications increased (74% vs. 10%, p < 0.01). Conclusion. The results of this study demonstrate the importance of careful management of obstructed labor to timely prevent the development of adverse maternal and neonatal outcomes.
