Relevance: Genital endometriosis is one of the most common gynecological disorders among women of reproductive age, significantly impairing their quality of life and negatively affecting fertility. Studying this pathology’s clinical and morphological characteristics helps improve diagnostic methods and develop optimal therapeutic strategies. Objective: To assess the clinical and morphological features of genital endometriosis in women of reproductive age. Materials and Methods: A retrospective clinical and morphological study involved 100 female patients aged 22 to The diagnostic approach included clinical examination, ultrasound imaging (US), magnetic resonance imaging (MRI), and laparoscopy with biopsy. The morphological evaluation included identification of endometrioid glands, and assessment of inflammation and fibrosis severity. The results were analyzed using statistical methods to determine the sensitivity and specificity of the diagnostic procedures. Results: The most common symptoms were pelvic pain (85%), dysmenorrhea (78%), and infertility (42%). Histological examination confirmed the presence of endometrioid heterotopias in all cases, along with signs of chronic inflammation and fibrotic changes in over half of the patients. The diagnostic effectiveness was 98% for laparoscopy with biopsy, 89% for MRI, and 72% for ultrasound. Conclusion: The findings indicate a high prevalence of pain and infertility in genital endometriosis and highlight the importance of morphological confirmation of the diagnosis. A comprehensive approach to diagnosis and treatment, incorporating highly informative imaging methods and combination therapy, significantly reduces the risk of recurrence and improves reproductive outcomes in women of reproductive age.
